STEPS Program: A Collaborative Approach to Transitional Care for Older Adults in Saskatchewan

The STEPS (Short Term Enablement and Planning Suites) program is an innovative initiative aimed at providing transitional care for older adults who no longer require acute hospital care but are not yet ready to return home or move into long-term care. The program is currently being piloted at Diamond House in Warman, Saskatchewan, in partnership with the Saskatchewan Health Authority (SHA). It offers up to 30 alternate-level-of-care (ALC) beds designed to support patients as they transition from acute care environments to more suitable, long-term living arrangements.

Program Objectives:
The primary goal of STEPS is to create a bridge between acute care and home, offering a supportive, lower-intensity care setting that allows patients to recover, regain abilities, and plan their next steps.
