In order to be diagnosed with scoliosis, you need to have an unnatural sideways curvature in the spine with a rotational component with a Cobb angle measurement of 10 degrees or greater. Scoliosis is measured on complete spine X-rays.
The Cobb angle measurement allows us to classify the severity of the scoliosis as follows:
Mild: Cobb angle measurement of 10-25 degrees
Moderate: Cobb angle measurement of 26-40 degrees
Severe: Cobb angle measurement of 40-80 degrees
Very severe: Cobb angle measurement 80 degrees or greater
Where curves go from moderate to severe, there is a variation between doctors and countries; in some instances, severe is determined after the curve reaches 40 degrees, while in others, it is after they pass 45 or 50 degrees.
Scoliosis is progressive; all mild curves have the potential to become moderate, and all moderate curves can become severe. Where scoliosis is initially diagnosed is no indication of where it will stay. The majority of cases will progress either rapidly during growth or slowly in the adult stage and accelerate as the patient gets older. The goal should be to slow down the progression and keep curves as low as possible so that mild curves never become moderate and so forth.
Only proactive treatment can counteract the progressive nature of scoliosis. Traditional treatment only suggests watching and waiting.
In adults, the most common symptom is back pain at the location of the curve because the nerves are compressed. They can also feel radicular pain in the legs and feet, and it can also happen in the arms if the scoliosis is higher. They can also experience hip and knee pain.
In children, however, the most common symptom of severe scoliosis is posture deviation. Uneven shoulders, hips, waistline, tilted shoulders and arms, and don't seem to hang their arms at the same length, prominent leans or tilt to one side or another. Children do not commonly experience pain.
Traditional treatment is commonly just watch and wait until the curve becomes severe enough to warrant surgery to treat the scoliosis and make the spine straighter. This includes using rods and screws to fuse the spine together and make it straighter using surgery. In a growing child, the surgery is usually done quickly, and they function adequately for the next 10-15 years. In adults, they're not as prone to surgery since the results are unpredictable, and there is a higher incidence of negative output.
To qualify for spinal fusion surgery, two requirements must be met: the patient needs to have severe scoliosis, and they have to be in a dire situation, such as severe pain where the risk of surgery outweighs how bad they're doing.
Conservative treatment combines chiropractic care, in-office therapy and rehabilitation, corrective bracing, and customized at-home exercises, and the goal is to prevent progression as soon as the scoliosis is found. This can be achieved by making the curve smaller, and by doing so, we can influence how much they progress in the adolescent stage, and we can also influence how much it progresses in the adult stage as smaller curves progress less as a response to gravity.
Regardless of the severity of your scoliosis at The Scoliosis Reduction Center, we recommend treating it as soon as possible as we can prevent invasive surgery later on if we can stop curves from processing. We offer conservative treatment that works at preventing progression, but most importantly, even if you already have severe scoliosis, in many cases, we can reduce the curve either below or very close to the surgical threshold.
