When coding for skin lesion excisions for ranges 11400 - 11646, there's a few steps to follow. The excision must be through the dermis. The code size is based on the widest diameter of the lesion plus the narrowest margins. Simple closure is included. You can then select the code based off the size, location, and if the lesion is benign or malignant.
