Video Not Working? Fix It Now

On January 31st, 2001, there was near-collision incident between JL958 and JL907 near Yaizu offshore, Shizuoka, Japan, at altitude of about 12,000 meters. It was said that they came as close as less than 200 meters in distance. Why did this major incident happen? I am going to explain it based on the incident report created by the Investigation Committee.
